Monrovia, Liberia – February 21, 2025 – The Environmental Protection Agency (EPA) has imposed a $5,000 fine on Sethi Ferro Fabrik Steel Factory following a fatal explosion at its oxygen plant in Gardnersville on February 17, 2025. The explosion, caused by an oxygen cylinder blast, tragically claimed the life of Saah J. Samukai, a worker at the facility.

An EPA investigation revealed serious safety violations, including:

Lack of fire safety measures

Improper handling of gas tanks

Missing emergency exits

Expired environmental permits


Due to these violations, the EPA ordered the immediate closure of the plant on February 19.

Despite the breaches, the EPA recognized the factory’s role in supplying free medical oxygen to hospitals and has agreed to issue a temporary 90-day permit once the fine is paid and safety improvements begin. However, a full environmental audit must be completed before full operations can resume.

The EPA has vowed strict monitoring to ensure compliance with safety and environmental regulations, reinforcing its commitment to workplace safety and environmental protection.
